Implement an action plan
A detailed, workable plan to improve long term well-being is expected and should include some actions that are critical for addressing the nutritional issue. Implementing the strategies/actions should involve interactions with a societal group affected by the issue to enable an evaluation to be completed.
The development and implementation of the action plan can be a group activity (working collaboratively) to enable a range of strategies/actions to be implemented. This may provide more opportunities for reflection.
The effectiveness of the action should be evaluated (assessed individual activity) by reflecting on the implementation of the plan, i.e. barriers and enablers; on-going refinement with reasoned examples; benefits and limitations of action(s); reflection of own feelings and those affected by the issue; conclusions drawn on how effective the actions were in terms of enhancing the well-being of the group with the issue.
Evidence is expected to support the implementation and evaluation, for example, a diary or logbook recording their individual participation in the action plan and their on-going reflection, detailed examples of observed activity, client or key stakeholder(s) responses, surveys, connections to the determinants of health that have contributed to the issue.
Implement an action plan with reflection
For Merit, critical reflection should be on the effectiveness of the plan and be supported by reasoned evidence. Consideration should be given to actions that have resulted in a positive effect on people’s well-being; the next steps or additional action to maintain the long term effects on well-being and/or the failure of some strategies to achieve the overall goal.
Implement an action plan with insight
For Excellence, a critical reflection with insight should include connections between the strategies/action and how effective they are in addressing the determinants of health, attitudes, values and key stakeholders.
Insight should be shown into the effect on the well-being of the group and further possible changes that may be needed to ensure more equitable outcomes for everyone in society. For example, how do attitudes and values contribute to the problem or help to address it, what are the possible risks or barriers to sustained long term change.
